If you believe that there has been a violation of your copyright or trademark on our site and you want us to take down or disable the material in question, you must provide us with all of the following information:

2. Identification of the copyrighted work or trademark you claim is being
infringed, or, if you want to cover multiple copyrighted works or trademarks in a single
notice, a representative list of such works or trademarks.
3. Identification of the material that you claim is infringing or is the subject of
infringing activity and that you believe should be removed or access to which should be
disabled, with information reasonably sufficient to permit us to locate the material.
4. Information reasonably sufficient to permit us to contact you, such as an
address, telephone number and, if available, an electronic mail address at which you may
be contacted.
5. A statement that you have a good faith belief that use of the material in the
manner complained of is not authorized by the copyright or trademark owner, its agent, or
the law.
6. A statement that the information in your notification is accurate, and under
penalty of perjury, that you are authorized to act on behalf of the owner of an exclusive
right that is allegedly infringed.
